New Patient Information

We can’t wait to meet you in our new facial plastic surgery office in the San Francisco, North Embarcadero area at 55 Francisco Street, Suite 705, 94133.

As part of ongoing practice improvements, we are now using electronic medical records and are able to offer you the convenience of entering your demographics and medical history information online prior to your appointment, and at your leisure. This will save you significant time at your upcoming appointment.  If you do not have a chance to fill out this information prior to your appointment, please arrive 15 minutes before your scheduled appointment.

Additional forms requiring a signature will have to be completed in-office.

New & existing patients, please click the My Patient Visit icon below.

You will need to enter your email address (same as what you provided to the office when making your appointment), First Name, Last Name (same as name on insurance card) and Date of Birth. In order for you to gain access to the new Patient Portal your permission is required. Please call the office for your security code or alternatively email jamie@drkimfacialplastics.com. For your convenience and to protect patient security we simply recommend you copy, paste and personalize the following into your email text:

“My name is ______”. My DOB is _______. I give permission to be registered for the patient portal. I provide confirmation for representatives at Dr David W. Kim to email me my access code.”

Once you have successfully logged into our portal, you will have options at the top for:

myInfo – allows you to update your demographics and insurance

myVisits – allows you to enter your medical history in forms or access a clinical summary from a previous visit

myMessages – allows you to send a secure message to the office

myAccount – allows you to link other family member accounts together or link with another medical office using MyPatientVisit.
